of any network, so you can go to any dentist, anywhere in the country. They don’t have to “take Aflac
Dental” or “be in the Aflac Network“ because there is no network, or requirement for a dentist to be a
participant in any network. If they take money, they are in our network - lol!

IMPORTANT -PLEASE NOTE: All Aflac Dental Plans, like most Indemnity dental plans, have waiting
periods for certain classes of services. There are services which you will not be able to use until
you have had the policy for 3 months, 6 months, or 12 months. Once you have met each waiting
period, those services are covered. Waiting periods only apply to the first year of coverage and do
not reset each year. Please see the Aflac Dental Plan brochures for details on the waiting periods
for each service.

In the following pages, you will find brochures for the 3 Aflac Dental Plans available to employees of
National Billing. Please note that the premiums for all Aflac Dental Plans are “Pre-Tax” qualified, so they
will be taken out of your paycheck on a “pre-tax” basis which will save you an additional 25-30% off
the listed group rates.

So, for example, the bi-weekly premium for Aflac Dental Essentials- Individual Coverage is $10.74/pay.
Since it is coming out of your paycheck on a “pre-tax” basis, the actual net change in your take-home pay
won’t be -$10.74, it will only be approximately -$8.05 per pay, or less, depending on your tax bracket.
That is the beauty of pre-tax qualified, payroll deducted benefits! You can thank your employer for
setting that up for you!
