SEARCH POLICY AND CONSENT TO SEARCH FORM

I, ______________________________, understand and acknowledge that the LSBA, in the interest of
the safety and health of its Employees, desires to inspect and search, at random and at announced
times, all packages, boxes, clothing, or any of my personal belongings carried on or off LSBA
property. I consent to these searches and agree to allow and cooperate with such inspections while
employed with the LSBA. If I refuse, I understand that I may be terminated.

I, ______________________________, hereby acknowledge that I have read the LSBA’s Alcohol and
Drug Screen Policy and consent to search and fully understand and consent to abide by them. I further
agree to abide by the policies and acknowledge that compliance with the policies is a condition of my
at-will employment with the LSBA. I also understand and have been advised that I should not rely on
any attempted oral modifications or amendments of the policy and that the only modifications or
amendments which are binding are those which are in writing and authorized by the LSBA Executive
Director.
