CONSENT FORM FOR SUBSTANCE SEARCHES AND TESTING

The misuse of legitimate drugs, or the use, possession, distribution, or sale of illicit or non-prescription
controlled drugs or paraphernalia is PROHIBITED. The possession, use, distribution, or sale of
alcoholic beverages or firearms are STRICTLY PROHIBITED on LSBA premises, any LSBA job site,
or any location where the LSBA is conducting business. No personnel may possess any of the above
mentioned items on their person, in their personal possessions that they have with them, or while on
LSBA business and/or LSBA property.

It is further agreed and acknowledged by my signature below that the LSBA may, at its discretion,
perform regular or unscheduled searches of myself and any persons entering the LSBA premises or job
site.

Upon finding any of the above-prohibited items, appropriate disciplinary action will be taken.
Immediate termination, expulsion from the LSBA workplace, and confiscation of the prohibited items
may all take place. It will be to the LSBA’s discretion as dictated by applicable law to press legal
charges and/or to refer the matter to appropriate authorities and to take appropriate disciplinary action,
up to and including discharge. .
