Due at the Field Trip:
 Field Trip Envelope with final money drop & field trip sheet - Money must be correct and enclosed in the envelope with all of the participants on your field trip listed on the sheet.
 Attendance Report - Must be completed with accurate cumulative site numbers & attendance totals for the week. This form must be filled out correctly in order to receive credit.
 Contest Reports - Turn in reports for all contests held, not just the Contest of the Week.
 Supply Request Forms – Please be specific with your counts, Ex) 50 pipe cleaners, 4 glue sticks, etc.
 Equipment Request Forms – Please write what day & time you need to use one.
 Completed Bus Exemption Forms-must include pick up person’s driver’s license number, please turn in to your
Supervisor before you get on the bus.
Additional Record Keeping
 Bulletin Boards – Information board must be updated by the end of the day Thursday, for the upcoming week, with all necessary weekly information. Boards must also be in good repair at all times and in a visible spot for the parents & participants to see when they arrive at the site.
 Emergency Forms - These forms (i.e., personal injury, property damage, theft, incident, etc.) should be turned in to the office before the end of the day in which it occurred. Indicate serious situations when turning it in so that they can be immediately followed up by the Director or Assistant Director.
Weekly Requirements
Your lesson plan can have no more than 3 doubled up activities. For example, the special event can be themed with the theme of the week and count for 1 theme activity and the special event. Please do not triple up your activities. This will be strictly monitored by your supervisor.
 Theme – Have fun with each theme! Refer to the Theme page for more details on each. Your activities should be different, and obvious as part of the theme. Simply re-naming an activity to fit the theme does not count. Plan & execute 5 theme activities for the week
 Craft Organization - Weekly craft samples must be visible, site has a designated display area, tables are covered & prepared BEFORE the activity starts, provide strong supervision & another simple activity is going as projects are finished. At least one different offering each day. Strong & consistent each day. There should be 3 crafts for the week.
 Contest Participation - Promotion is key to participation. The more participants, the more fun the contest will be. Allow as many winners as possible. Make up & promote contest categories early in the week so the children know what they are working for. The use of different age groups & at least four creative categories
 Extra Contest - Offering different contests on a weekly basis (i.e., hula hoop, limbo, nok hockey, coloring, etc.) Winners should be recognized & contest reports must be turned in as well. Plan & execute at least one contest besides the provided weekly contest
 Special Events – This is a one-time activity. Ideas: Dance contest, special field trip/party, lip sync, car wash, skits, etc. An activity is not a special event if it is offered week after week. Be unique and original! Offer one unique activity each week
 Health & Fitness- Encouraging health & fitness into the lesson plan is very important. Please come up with unique & creative activities for the entire summer. (Fitness stations, nutrition trivia, etc.) Plan & execute at least two health & fitness activities for the week.
Field Trips
 Active Supervision - Staff members must be interacting with participants who are behaving appropriately and are under the supervision of a staff member at all times. Have a list of participants for each staff person that includes emergency phone numbers.
 Wrist Bands - Each child in attendance must have a visible wrist band with their name printed on it. Both the child’s name & the site name must be legible using a dark, permanent marker. Bring additional wrist bands to replace any that slip off.
 Rosters & First Aid Supplies - In the possession of each staff member at the field trip.
 Bus Signs - Must have 4 signs with site name visible in the bus windows. Please bring tape with you (provided for you
in your craft bin) to put the signs on the windows. We recommend you create your signs on light colored paper
because bus windows are tinted. Ex) yellow, light blue, bright orange.
 Promptness - To arrive promptly & be at the designated spot on time ready to leave at the predetermined departure
time. This means that your area is cleaned up, stuff put away & souvenirs purchased before the departure time.
 Permission Slips – Do you have a signed permission slip for every participant? Did you turn them in to your
supervisor in order by your field trip sheet? PAGE | 89 2018
