                When the survey concludes, remind Jim that he can talk with


                his case manager/care coordinator about getting the ramp


                fixed




                Surveyors should follow the state’s unmet need protocol, and


                at minimum remind the person that the case manager/service


                coordinator is a good point of contact to discuss needs.




                Follow-up should always occur after the survey is complete.
