• Frank does not communicate verbally – he typically uses finger spelling, but

you are unable to understand his communication. His support worker
Susan can be present for the meeting.

• Taylor answers the proxy determination questions validly during the Initial

Contact, but at the start at of the meeting they have difficulty answering

questions. Taylor’s guardian is present for the meeting.

• After completing the first section of the survey Tonya gets another call and
