Q11 Question
After completing each survey you should:
• Throw out any papers with identifying information about the
person you surveyed
• Complete the Surveyor Feedback sheet
• Talk to the person's case manager/care coordinator or staff about
issues the person discussed with you
• Tell a family member about a funny story the person shared
• Talk to your coordinator about any concerns you have about the
survey you conducted
