Complete the Surveyor Feedback sheet; Talk to your
coordinator about any concerns you have about the survey
you conducted
Surveyors should never share information from the survey
meeting with anyone except their NCI coordinator without
consent (with the exception of mandated reporting issues).
Surveyors should follow their state protocol on what to do
with any papers that contain personally identifying
information.
