Important Reminders during the enrollment process:
• Actively elect all benefits you wish to be enrolled in for 2020

• Add/update your life insurance beneficiary, if eligible as a full-
time employee

• Open your Health Savings Account, if you are eligible and
electing a GESMN medical plan for 2020.
• Complete your Evidence of Insurability (EOI) form if electing HEALTH PLAN

Changing your Elections Mid-Year

While the benefit choices made remain effective for the entire year DENTAL PLAN
of 2020, there are special circumstances when changes are allowed, The Standard

If you experience a qualified life event, contact benefits
administration as soon as possible but no later than 30 days of the
event. Events that occur after 30 days will not be considered for a FLEXIBLE BENEFIT PLAN
