Fall Assessment
It is part of the core assessment that provides rapid
method of assessing an individual’s likelihood of falling,
generates a score to identify the level of care needed
and identify the most likely contributing factors.

Once the patient’s care needs have been identified,
referrals will be made to the relevant professionals and
service providers to develop an individualised care plan:
For example, the ReGen Hospital will provide a care plan
to modify risk factors for inpatients. Meanwhile, the care
plan in the community will focus on involving the relevant
authorities to provide any necessary service and
supplying carer information leaflets.
