TEXAS MUTUAL INSURANCE COMPANY DENTAL PPO PLAN
The Plan's Benefits are administered by Texas Mutual Insurance Company, the Plan
Administrator. UnitedHealthcare Dental is the Claims Administrator and processes
claims for the Plan and provides appeal services; however, UnitedHealthcare Dental and
Texas Mutual Insurance Company are not responsible for any decision you or your
Dependents make to receive treatment, services or supplies, whether provided by a
Network or non-Network Dentist. UnitedHealthcare Dental and Texas Mutual Insurance
Company are neither liable nor responsible for the treatment, services or supplies
provided by Network or non-Network Dentists.
