The Claims Administrator is allowed to disclose ID, NH, NV
prescription-related information only (1) under
certain limited circumstances, and /or (2) to
specific recipients.
Communicable Diseases
The Claims Administrator is allowed to disclose AZ, IN, KS, MI, NV, OK
communicable disease information only (1) under
certain limited circumstances, and /or (2) to
specific recipients.
Sexually Transmitted Diseases and Reproductive Health
The Claims Administrator is allowed to disclose CA, FL, IN, KS, MI, MT, NJ, NV, PR, WA, WY
sexually transmitted disease and/or reproductive
health information only (1) under certain limited
circumstances and/or (2) to specific recipients.
Alcohol and Drug Abuse
The Claims Administrator is allowed to use and AR, CT, GA, KY, IL, IN, IA, LA, MN, NC, NH,
disclose alcohol and drug abuse information (1) WA, WI
under certain limited circumstances, and/or
disclose only (2) to specific recipients.
Disclosures of alcohol and drug abuse information WA
may be restricted by the individual who is the
subject of the information.
Genetic Information
The Claims Administrator is not allowed to CA, CO, KS, KY, LA, NY, RI, TN, WY
disclose genetic information without your written
consent.
The Claims Administrator is allowed to disclose AK, AZ, FL, GA, IA, MD, ME, MA, MO, NJ, NV,
genetic information only (1) under certain limited NH, NM, OR, RI, TX, UT, VT
circumstances and/or (2) to specific recipients.
Restrictions apply to (1) the use, and/or (2) the FL, GA, IA, LA, MD, NM, OH, UT, VA, VT
retention of genetic information.
HIV / AIDS
The Claims Administrator is allowed to disclose AZ, AR, CA, CT, DE, FL, GA, IA, IL, IN, KS, KY,
HIV/AIDS-related information only (1) under ME, MI, MO, MT, NH, NM, NV, NY, NC, OR,
certain limited circumstances and/or (2) to specific PA, PR, RI, TX, VT, WA, WV, WI, WY
recipients.
Certain restrictions apply to oral disclosures of CT, FL
HIV/AIDS-related information.
The Claims Administrator will collect HIV/AIDS- OR
related information only with your written consent.
