LIFE INSURANCE

Our life insurance benefit is provided
free to full-time employees. The benefit
is 1.5X base salary up to $750,000.

Employees may purchase additional life
insurance for themselves and for their
spouse, up to $500,000. They may also
purchase child life insurance of $10,000
for children up to age 26.

CANCER INSURANCE

Employees can purchase Aflac cancer protection
assurance for themselves, their spouse and their
children up to age 26.

Aflac cancer assurance provides financial benefits
in the event of a cancer diagnosis and treatment.

FLEXIBLE SPENDING ACCOUNTS

Flexible spending accounts allow employees Care Type Plan Link
to set aside pretax money to pay for eligible
healthcare and dependent care expenses FSA myuhc.com
within the calendar year.

• Healthcare FSAs can be used to pay for
eligible expenses such as doctor copays,
prescriptions and contact lenses

• Dependent care FSAs can be used to be
reimbursed for qualifying child care and
elder care expenses.
