Livongo® for Diabetes Program
If you are enrolled in the HSA or PPO medical plans and you or your covered family members have diabetes and meet the program requirements, you are eligible to participate in the Livongo for Diabetes Program at no additional cost.
Take advantage of this customized program to help make living with diabetes easier. If you qualify, you’ll get these tools delivered to your home when you join:
• An Advanced Glucose Meter ($200 value) • Lancing Device
• Lancets
• Charger
• Carrying Case
Livongo participants also enjoy:
• Personalized web portal to access your personal health account
• Diabetes education
• Access to Livongo’s cellular, mobile and web-based diabetes management systems and technologies
• 24/7 on call, monitoring and support by Livongo’s Certified Diabetes Educators (coaches)
• Unlimited blood glucose monitoring supplies listed above, as requested (does not include diabetes medications such as insulin and pills) at no cost to the member

You Must Call Before ObtainingThese Services
If you are enrolled in a non-HMO medical plan, you already know you must call a health advocate before scheduling any outpatient non-emergency CT scans or MRI services. In 2019, this list is expanded to include:
• Imaging and CT scans • X-rays
• Joint replacement
• Bariatric surgery
• Musculoskeletal IP/OP procedures
• Breast reductions (medically necessary)
Failure to call before receiving the following services will result in a $200 penalty.
Emergency Room Tiered Copay
Your first step when you become ill should be to contact your regular doctor — one who keeps your medical history on file and will see you in a pinch.
As a back-up, urgent care centers can treat non- emergent issues faster and cheaper than a hospital Emergency Room (ER). For those times when an ER is truly necessary, we’ve established a tiered ER copay program to encourage wise emergency room use for PPO members in 2019:
• Visits1-3:$200copay,then20percent;
• Visits4-5:$250copay,then20percent;
• Visits6+:$300copay,then20percent;
• Non-emergentERvisitssubjecttocopay, then 50 percent.
