form for purposes of underwriting.

Ans. The headings in the Public Liability Risk Assessment
form are:
(i) Particulars of Insured and Management of the
Company;

(ii) General details of Premises;

(iii) Particulars of assets on Premises.

(iv) Effluent Discharge - Ref of Q1 of Nov 2007
whether discharged from the Plant outside the
premises by pipeline;

Whether effluent treated before discharge
according to Pollution Laws;

Whether coverage required, if so, the length of
pipeline from the Compound Wall of Premises to
the disposal point.

(v) Compulsory Insurance - whether insured under
Public Liability Insurance Act

If so, name and address of the insurance company
and policy number (certified copy of the premium
receipt to be furnished).

(vi) Pollution coverage - production programs and main
products, mode of production - continuous or batch,
