Compensation for occupational diseases may be
covered at an additional premium of 50% of the
standard rate.
The policy may be extended to indemnify the insured
in respect of the medical, surgical and hospital
expenses incurred by the insured for any occupational
illness or injury.
Q5. Specify any 8 major items of information in the
proposal form for public liability insurance.
Ans. Some of the major items of information in the proposal
form are :
(a) Declaration that the statements and particulars are
true.
(b) The name and business address of the proposer.
(c) Full particulars of the proposer's trade or occupation
with precise description of work.
(d) The Schedule of employees, showing the estimated
number and the estimated annual wages, salaries
and other earnings.
(e) If the proposer desires to insure his liability under
the Workmen's Compensation Act, 1923, he has to
provide the name of contractors, nature of work
sublet and estimated amount of contract.
