Dear Mr. Doe:
Your Medicare evaluation is enclosed. Please check the information on the client data sheet on
page 5 to make sure it is correct. You should enroll in your supplemental coverage no later than
the second week of February. That way, you will have your enrollment cards before March 1.
And if you haven’t done so yet, you can enroll in Part A and Part B by calling Social Security at
(800) 772-1213.
Once you’ve selected the supplemental coverage you want, you can enroll in one of the
following ways:
To enroll in an Advantage plan, you can call either the plan’s toll-free number,
shown on pages 2-3 of your evaluation, or (800) MEDICARE. Make sure that your
coverage will begin on March 1. You may need new prescriptions from your
doctor(s), but first ask if your current prescriptions can be transferred to the plan
you’re enrolling in.
If you decide to get a Medigap policy and a Part D stand-alone plan, you will
likely need to make several calls. It’s a good idea to call a minimum of three
insurance companies to get their current Medigap quotes before choosing the
company to purchase your policy from. When you decide on a company, make
sure that your policy’s coverage starts on March 1. The companies’ toll-free
numbers and most recently published premiums are shown in Appendices A1,
A2, and A3.

number, shown on page 4 of your evaluation, or (800) MEDICARE. Make sure
that your coverage will begin on March 1.
You indicated that you would like to see comparisons of Advantage plans and Medigap policies.
The evaluation compares two Medigap plans (Plan F and Plan L) and two Medicare Advantage
HMO plans. Both Advantage plans have good quality ratings and low costs for the prescription
drugs that you take. Also, your doctors are listed in both Advantage plans’ networks, although
prior to enrolling in a plan, you should verify that your doctors are still in its network.
