Here are summaries of the two Advantage plans compared in your evaluation.

                   1)  Humana Gold Plus HMO Plan. This plan has low costs for your prescription drugs and
                       does not have a health plan deductible or premiums. It also has a network of more than
                       3,500 providers, including the three doctors that you see, and it has an above-average
                       four-star quality rating from Medicare (five stars is best). And if you get mail-order
                       refills, it has zero costs for the prescription drugs you take.

                       Probably this plan’s weakest feature is its $5,900 out-of-pocket limit, which is high for
                       an HMO. Co-payments for an office visit to your primary care doctor are $10 and $50 to
                       see a specialist. In all Advantage plans, recommended preventive tests are 100%
                       covered.

                       AARP Medicare Complete Plan 1 HMO Plan. This plan also has zero premiums for
                       health coverage, no health plan deductible, and a Medicare-quality rating of four stars.
                       Co-payments for an office visit to your primary care doctor are $10 ($45 to see a
                       specialist). This plan’s $5,500 out-of-pocket limit is $400 lower than that of the Humana
                       Gold Plus HMO Plan.

                       One weak point is the size of this plan’s provider network, which is roughly one-third the
                       size of the network for the Humana Gold Plus HMO Plan.

               Rx Drug Coverage

               If you purchase a Medigap policy you must also enroll in a Part D stand-alone plan. Please check
               the list of your Rx drugs, dosages, and monthly quantities that’s attached to this letter to make
               sure they are correct (this list was used to find the lowest-cost plans). The drug costs shown
               below and on page 4 of your evaluation are for the entire 2018 plan year, and they include
               premiums, deductible, and co-payments. Your costs will be slightly less since you’ll be enrolled
               for only 10 months.

               For the drugs that you take, the lowest-cost stand-alone plan is the Envision Rx Plus
               Prescription Drug Plan. If you get mail-order refills, annual costs for your drugs are an estimated
               $187 – again, that includes premiums, deductible, and co-payments. CVS is a preferred
               pharmacy in this plan, and if you need short-term prescriptions filled you can go to CVS, Rite-
               Aid, or Sav-On, all of which are preferred pharmacies, to get the lowest prices.

               If you decide to enroll in either of the Advantage plans in your evaluation, your Rx drug
               coverage is included in your benefits and you cannot unbundle it from the health coverage.  In
               the Humana Gold Plus HMO plan, there are zero costs for the entire year if you get mail order
               refills. CVS is a preferred pharmacy in this plan.

               In the other Advantage plan – the AARP Medicare Complete Plan 1 – the estimated 2018 cost
               for your drugs if you get mail-order refills is $204. In this plan, there are no co-payments for the
