               Dear Hans:

               Your Medicare evaluation is enclosed. Your estimated costs for the last three months of 2021
               are shown on page 3 of the evaluation, and your annual estimated costs are explained later in
               this cover letter.

               As we discussed, it’s best for you to choose a Medigap policy and stand-alone drug plan for
               your Medicare supplemental coverage. You indicated that you may soon re-locate to another
               area, and regardless of where you move in the United States, you will be covered by a Medigap
               policy. Then at some later time if you wish to switch to a Medicare Advantage plan, you can do
               that during each year’s annual enrollment period (October 15 – December 7).

               Since you already have Part A and Part B, the remaining three steps are to:

                       1)  Enroll in a Medicare Prescription Drug Plan, also called a stand-alone drug plan.
                          When you do this, you will be enrolling in Part D. Even though you do not take any
                          prescription drugs, you must enroll in a drug plan to avoid lifetime penalties.

                          The lowest-premium stand-alone drug plan in Alameda County is the SilverScript
                          SmartRx Plan with a $7.20 monthly premium. Walgreens, the pharmacy you use, is
                          not in this plan’s network. So, when you do need a prescription filled, you will likely
                          get the lowest prices at a CVS pharmacy, which is a preferred pharmacy in this plan.
                          To enroll, call the plan at (833) 526-2445. You will likely be asked to show evidence
                          that you’ve had creditable Rx drug coverage since you turned 65. This is a national
                          plan that you can use in any state.

                       2)  Decide which Medigap plan you want. Your evaluation compares two plans –
                          Medigap Plan G and Medigap Plan N. These are both comprehensive plans that
                          provide good coverage.

                       3)  Decide which insurance company you will get your Medigap policy from. The phone
                          numbers of the companies that sell Medigap policies in California are shown in
                          Appendix A1, and the premiums for Plans G and N are shown in Appendices A1, A2,
                          and A3. If possible, you should sign up for a plan within the next few days so that
                          you will have your membership card by October 1.
