These estimated drug costs may be higher than you will pay because they assume you will get
refills on all your prescriptions on January 1 – but you will more likely get refills after your
existing supplies run low. If you can save money by getting your refills in your current employer
plan, you can try to get as many refills as your employer plan’s rules allow.

Also, try to use your new plan’s preferred pharmacies if possible. Generally, prices are lower at
a plan’s preferred pharmacies, which are listed on page 4. I was unable to find a preferred
pharmacy in your zip code for the AARP Medicare Advantage Plan 1, and it’s possible that it
does not have any preferred pharmacies (some plans don’t). Still, if you enroll in this plan it
might be worth checking with the plan to see if it has any preferred pharmacies in your area.

You asked how much your drugs would cost without Farxiga and Victoza. There’s no simple way
to know this because of Part D’s complex design. A single drug can have as many as four
different prices depending on which of the four coverage phases it’s purchased in – 1) the cost
in the deductible phase, 2) the cost after the deductible is met but before the coverage gap, 3)
the cost in the coverage gap, and 4) the cost in the catastrophic coverage phase (after the
               coverage gap).

So, the following are very rough estimates. According to the Medicare Plan Finder program,
here are your costs with Farxiga and Victoza and your costs without these two drugs at a CVS
pharmacy in each of the three plans:

AARP Medicare Advantage Plan 1 (HMO-POS)
Costs of all your Rx drugs $2,948
Minus costs of Farxiga and Victoza -$2,487
Your Rx drug costs without Farxiga & Victoza $461

Optima Medicare Value Advantage HMO Plan
Costs of all your Rx drugs $2,973
Minus costs of Farxiga and Victoza -$2,421
Your Rx drug costs without Farxiga & Victoza $552

               HMO Choice PPO Plan
Costs of all your Rx drugs $3,115
Minus costs of Farxiga and Victoza -$2,643
Your Rx drug costs without Farxiga & Victoza $472

Attached to this letter is a comparison of three Advantage plans’ costs for Farxiga and Victoza.
Since none of the three plans have premiums for their drug coverage, the costs shown are all
               co-payments and co-insurance payments.

Vic, please let me know if you have any questions. There are a couple of things to keep in mind
as you make the transition to Medicare as your primary coverage. On your initial visits to your
new doctors after January 1, show the office staff your enrollment cards for Medicare A&B and
