Benefit Plan Eligibility and Enrollment




Eligibility

You will become eligible for insurance on the day you You may be required to provide a copy of one or
complete the waiting period if: more of the following documents to support your

X You are in a class of eligible employees dependent’s eligibility.

X You are an eligible, full-time employee X Marriage certiicate or license

X You normally work at least 30 hours week X Birth certiicate for each biological child
X You pay any required contribution X Birth certiicate for each stepchild showing your
spouse as the child’s parent
Documents to Support X Final adoption certiicate
Dependent Eligibility X Legal adoption agency or placement certiication

The Carlstar Group is entitled to perform periodic X Legal document for court-appointed guardianship

audits to verify dependent eligibility.




Spousal Coordination of Benefits Policy

If you have a working spouse who is eligible for coverage under a medical plan through his/her employer, your
spouse is required to enroll in their medical plan unless your spouse’s employer requires your spouse to pay 100%
of the coverage. If your spouse’s employer does share in the cost of the medical coverage, and your spouse does
not elect to enroll in their employer’s plan, your spouse will not qualify
for coverage under The Carlstar Group medical plan and any related
claims will be denied. When your spouse is enrolled in another plan,

that plan is considered PRIMARY for your spouse and all claims
should be submitted to that plan.


Failure to comply with this policy will prevent your spouse
from being eligible for or covered by medical benefits
provided by The Carlstar Group group medical plan. Any
misrepresentation(s) regarding the eligibility or coverage of
your spouse may result in your immediate termination of
employment with The Carlstar Group.
