2020 Incentive
Associates employed prior to November 1, 2019 and who enroll in either Stein Mart medical plan for coverage
beginning January 1, 2020 are eligible to participate in the voluntary well-being incentive program. Through the
program, you could earn an annual reward of up to $1,200.
Complete a Free Health Screening Prior to November 1, 2019
Medical plan participants and covered spouses must complete a free Health Screening (blood draw) at LabCorp
before November 1, 2019 to qualify for the incentive.

You must bring the 2020 Voucher* and a photo ID with you to the appointment. For the most accurate results,
fasting for at least 8 hours is recommended. Fasting means no food or drink with the exception of water. Please
take any medications as normal.
Review Your Results
Reviewing your results is an important step to healthy living as it can identify areas of risk so you can take action
to improve your health. Understanding the results can sometimes be confusing; we recommend downloading the
LabCorp Guide* to explain each category. If any of your values fall outside of the desired range, we encourage you
to read the tips provided, and consider scheduling an appointment with your primary care physician.

Start Saving
Beginning in January of 2020, you will start receiving your incentive. If you are employed with Stein Mart and
covered under either medical plan through the entire calendar year, you will save a total of $600 for associate
completion, or $1,200 for associate and covered spouse completion. The way you receive your money depends on
your plan enrollment:
