Your Health Risk Assessment
Your Health Risk Assessment
The annual Health Risk Assessment (HRA) is a critical component of our plan
and we strongly encourage your participation. Mitchell Gold + Bob Williams has
partnered with Healthstat, Inc. to provide onsite services for the completion of your
annual Health Risk Assessment. The services in the clinic are provided at no cost to
our employees who are covered under our health insurance plan.
Clinic Hours
X Monday 8:00 a.m.–4:00 p.m.
X Tuesday 7:00 a.m.–3:30 p.m.
X Wednesday 8:00 a.m.–4:00 p.m.
X Thursday 7:00 a.m.–3:30 p.m.
X Friday 7:00 a.m.–11:00 a.m.
All employees and spouses covered under our health plan will need
to complete the HRA within 30 calendar days of your insurance
