Accidental Death and

Dismemberment Coverage

Accidental death and dismemberment (AD&D) coverage provides
protection if your death is the result of a covered accident. AD&D also
provides beneits for the accidental loss of hands, feet, eyesight, speech,
or hearing.

Base Plan

The AD&D Program provides coverage equal to 1 times your base annual
salary up to a maximum of $600,000. The entire premium for the Base
Plan is paid by Saint Louis University.

Supplemental Coverage

Employee Coverage
You may select additional levels of AD&D coverage in units of $10,000

up to $500,000 at the cost of $0.21 per each additional $10,000.

Dependent Coverage

Family AD&D coverage is also available with any 1 of the 3 following
options.

X Spouse only: your spouse’s beneit is equal to 50% of your selected
beneit

X Spouse and children: your spouse’s beneit is equal to 40% of your
selected beneit and 10% for each child
X Children only: your children’s beneit is equal to 15% of your selected
beneit
