Changing Your Coverage Eligibility and
Elections (including waivers) are binding for the entire calendar year.
During the plan’s open enrollment time period, which generally occurs Enrollment
in late Fall, you will have the opportunity to change your elections for
the following calendar year. The only way you can change your election
mid-year is if you experience a qualiied life event. A qualiied life
event will allow you to change your coverage election. Examples of a
qualiied life event are the following:
X Marriage
X Divorce or legal separation
X Birth of your child
X Death of your spouse or dependent child
X Adoption of or placement for adoption of your child
X Change in employment status of employee, spouse, or dependent
child
X Qualiication by the Plan Administrator of a child support order
for medical coverage
X Entitlement to Medicare or Medicaid
You must notify Human Resources within 31 days of the qualiied life
event. Depending on the type of event, you may be asked to provide
proof of the event. If you do not contact Human Resources within 31
days of the qualiied event, you will have to wait until the next annual
enrollment period to make changes (unless you experience another
qualiied life event).
