Additional Voluntary Life Details
Should you wish to enroll in an amount greater than $150,000 for yourself
or greater than $30,000 for your spouse, please complete the Statement of
Health Form (medical history statement) found on the NextCare Intranet
beneits page and email to Michelle Geeve.
If you decline this coverage when irst eligible, you may join at the next
open enrollment period but will be subject to completing the Statement
of Health Form for any amount of coverage elected. You may increase
your coverage amount by $10,000 up to $150,000 and your spouse’s
coverage amount by $5,000 up to $30,000 during each open enrollment
period if your salary allows. Increasing by up to $10,000/$5,000 does not
require a Statement of Health Form to be completed.
When enrolling in the voluntary life insurance beneit, you will be
provided the MetLife Travel Assistance, MetLife Identity Theft Solutions,
and MetLife Will Preparation beneits. Please see the NextCare Intranet
beneits page for more details.
