Enrollment and Eligibility



New Hire Enrollment

Welcome to our team! As a new employee, you will be eligible for
most beneits the irst day of the month following 60 days of full time
employment. The beneit enrollment process can be completed online

anytime from your date of hire through the 15 of the month prior to
your beneit start date. Payroll deductions are taken the same month
as coverage. If the beneit enrollment process is not completed by the
deadline, you will waive your rights to participation in the NextCare
beneit plans.


Annual Enrollment

Annual enrollment is your annual opportunity to review your current
beneits and make beneit changes for the upcoming plan year. During
annual enrollment, you can add, change, or decline coverage. In addition,

you can add and/or drop dependents during this time.


Mid-Year Enrollment Changes

Once you make your elections, you will not be able to make changes until
the next annual enrollment period unless you experience a qualifying life
event. A qualifying life event is a change in your personal life which may
impact your eligibility or dependent’s eligibility for beneits. Examples of
some qualifying events include the following.


„ Change of legal marital status (e.g., marriage, divorce, death of
spouse, legal separation)

„ Change in number of dependents (e.g., birth, adoption, death of
dependent, ineligibility due to age)


„ Change in employment or job status
You have 30 days from your qualifying event date to make your change.

If you do not make changes within 30 days you will need to wait until the
next annual enrollment opportunity. Contact Michelle Geeve in Human
