For many of us, nothing is more important than our health and the health of our family members. This is why we
offer a robust medical and pharmacy insurance program to our associates. We partner with Anthem to offer this
coverage.


Two Medical Options PPO Plan

All employees have a choice between two medical plan ‹ Offers higher per-paycheck premiums with lower
deductibles and out-of-pocket maximums
options. The two plans are respectively titled the PPO
Plan and the HDHP/HSA Plan. ‹ A meaningful option for those who prefer to pay
more in premiums throughout the year and less

New in 2021
‹ Virtual sleep consultation—The Live Health Online network is expanding to include sleep medicine specialists. Coverage
will include a virtual sleep medicine clinic and home sleep testing consultation.
‹ Physical therapy/occupational therapy—Separate oice visit limitations will no longer apply. Efective 1/1/21, members
will be eligible for 40 total visits, regardless of therapy type (physical or occupational).
‹ New ID cards will only be issued if a member switches between plans. If you elect to remain in the same medical plan
for 2021, a new ID card will not be issued. If you need a new card, you can call Anthem customer service or go to
www.anthem.com and download/print a new card.
