Eligibility

You are eligible for beneits if you:


„ Are a regular, full-time team member
„ Work at least 30 hours per week

Your dependents who are eligible for beneits are:


„ Your spouse or domestic partner

„ Your unmarried children who are:
ˆ Under age 26 (regardless of student or marital status for
medical coverage)

ˆ Permanently and totally disabled (if disability occurred prior
to age 26, as described above)

ˆ Under age 26 (regardless of student status) for all non-
medical lines of coverage
