Annual Enrollment
How to Enroll
For your 2016 Annual Enrollment, all team members will be utilizing
a web-based enrollment system which will allow you to record your
own elections for the new plan year. Please follow the steps below to
successfully complete your enrollment for the 2016 plan year.
Step 1: thoroughly review this guide prior to accessing the
enrollment system
Step 2: gather dates of birth, social security numbers, and the
correct name spellings of any new dependents that you will be
covering for the irst time
Step 3: access the enrollment system
Website: www.alacatwork.com
Username: 9-digit social security number (with or without
dashes)
PIN: last 4 digits of SSN + 2-digit year of birth
PIN example:
Full SSN = 499-98-9877
Date of birth 07/05/1983
PIN= 987783
Step 4: upon logging in you will be prompted to change your
PIN; your PIN can be alpha-numerical with a minimum of six
characters.
Step 5: review your personal and existing dependent information
for complete accuracy
Step 6: review your existing elections and complete your
enrollment for the 2016 plan year
Step 7: complete your enrollment by signing the Enrollment
Conirmation
Step 8: you will receive an email within one business day of the
completion of your enrollment certifying your elections
