2014 to November 25, 2014, your beneit coverage will be afected as
follows:
X You’ll have employee-only medical coverage under the Standard PPO
opion
X You won’t have any opional coverage (dental, vision, employee
supplemental life insurance, spouse life insurance, child life insurance,
health care and/or dependent care lexible spending account
contribuions, and voluntary Group Accident insurance coverage)
Making Changes During the 2015 Plan Year
Under IRS rules, your beneit elecions must remain in efect unil
the next plan year which starts on January 1, 2016 unless you have a
qualifying event during the year. You must register your changes by
visiing www.DANbeneitsplus.com or calling the beneitsPLUS soluions
center within 31 days of the qualifying event.
