offers you a range of beneit options. This beneits guide is an
important way we communicate with you regarding our beneits
program. Additionally, we hope this guide helps educate and
empower you to elect the insurance coverage and beneit support you
and your family need.
Annual Enrollment
It’s time to make some important decisions about beneits for the
2016 plan year. Annual enrollment will take place from November 9
through November 22. You will not be required to make beneit
elections for 2016. However, if you choose to participate in the
FSA, you must re-elect your contribution amount. Please review
this guide to learn more about your beneits. All elections placed
during this annual enrollment period take effect January 1, 2016.
How to Enroll for 2016 Benefits
Please log in at www.hcm.ininisolved.com to access your iSolved
account.
Eligibility
You may enroll in the beneits program if you are a regular full-time
employee who is actively working a minimum of 30 hours per week.
Your eligible dependents include:
Legal spouse, who does not have access to his/her own
employer-sponsored health plan
Dependent children up to age 26
Legal wards; children for whom you are a legal guardian, up to
age 26
