Beneits Enrollment
Hussmann Corporation provides you with the opportunity to
purchase a maximum critical illness beneit from $5,000 to $20,000
in $5,000 increments. In addition, you also may purchase a maximum
critical illness beneit of $5,000 or $10,000 for each covered child.
Critical illness insurance provides a beneit for the following illnesses
and conditions. Beneits are paid at 100 percent of the maximum
critical illness beneit unless otherwise stated. For a complete
description of your beneits, along with applicable provisions,
exclusions, and limitations, see your certiicate of insurance and any
riders.
