Life and AD&D Insurance
Monthly Rate for Child Beneit Basic Life Insurance and AD&D
$5,000 $0 .75 All full-time beneit eligible employees will receive a company paid

Spouse Flat amount of $10,000 NVW Newco, LLC with UHC offers a supplemental life insurance
or $20,000
Child(ren) Flat amount of $5,000 plan and AD&D plan to all employees. You have the option to elect
or $10,000 $10,000 increments up to $100,000. Amounts in excess of $80,000 will
require you to submit evidence of insurability. See below rate table.
You can also elect coverage on your spouse of $10,000 or $20,000. You
must elect coverage on yourself to cover your spouse and/or children.
You can elect $5,000 or $10,000 on children. The child rate covers all
children in the family.
