ADDITIONAL VOLUNTARY BENEFITS
Voluntary benefits are available for all regular full-time and regular part-time
employees working 20 hours or more per week.
Expenses, big and small, can add up. Think about your ability to pay for those
expenses if you or your family member were seriously injured as a result
of an accident, critical illness, or hospitalization. Cigna’s Accidental Injury,
Critical Illness, and Hospital Care insurance can supplement your health plan
and provide you and your family with the coverage and additional financial
protection you may need for expenses associated with an unplanned covered
accident, illness, or hospitalization. These plans pay benefits directly to you.
What you do with the money is all up to you.
Critical Illness Insurance
There are a number of non-medical expenses associated with a critical illness
and a major medical plan may not cover them all. Critical Illness coverage
helps you focus on recuperation instead of the distraction of out-of-pocket
costs. The Critical Illness plan pays you a cash benefit upon a diagnosis of a
covered illness. Covered diagnoses include but are not limited to the following:
Cancer Portable—coverage is portable
which means you can take this
Heart attack plan with you if you no longer
Organ Failure work for CareHere.
Stroke Health screening (wellness)
beneit—provides a beneit
Guaranteed issue—each year during open enrollment, you can elect this amount of $50 for a health
coverage without medical answering questions. screening test; the beneit
is limited to payment of one
Employee: up to $30,000 beneit amount per calendar
Spouse: up to $15,000 (maximum of 50 percent of employee-elected year for each covered person;
beneit waiting period is 30 days
amount) following the efective date of
Child(ren): 25 percent of employee-elected amount your critical illness insurance.
Employees must be actively at
work to elect this beneit.
