CRITICAL ILLNESS
Cigna
Critical illness insurance, available through
Cigna, is designed to help you ofset the
inancial efects of a catastrophic illness with
a lump sum beneit if you or a loved one are
diagnosed with a covered critical illness. The
critical illness beneit is based on the amount of
coverage in efect on the date of diagnosis of a
critical illness or the date treatment is received
according to the terms and provisions of the
policy.
During beneits enrollment, you can elect this
coverage without medical questions.
„ Team member: increments of $10,000 up
to $30,000
„ Spouse/domestic partner: increments of
$5,000 up to $15,000
„ Child(ren): increments of $5,000 up to
$15,000
„ Coverage is portable which means you
can take this plan with you if you no longer
work for the company
„ Your rates lock-in at your current age and
do not increase as you age
„ Diagnosis must occur at least 31 days after
the coverage efective date to be eligible
for beneits

Wellness Benefit
This beneit pays $50 per calendar year per
insured individual if a covered health screening
test is performed, including blood tests, chest
x-rays, stress tests, mammograms, and
colonoscopies. A full list of covered tests will be
provided in your certiicate.
Covered conditions (paid at 100% of elected
amount unless otherwise noted below)
„ Cancer
„ Carcinoma in situ (paid at 25%)
„ Heart attack
„ Coronary artery bypass surgery (paid at
25%)
„ End stage renal (kidney) failure
„ Major organ failure
„ Permanent paralysis as the result of a
covered accident
„ Coma as a result of severe traumatic brain
injury (covered at 25%)
„ Blindness
„ Benign brain tumor
„ Additional covered conditions for
dependent children: cerebral palsy, cystic
ibrosis, Down syndrome, and spina biida
