You will continue to be able to see any dentist; however, if you visit a
dentist in the CONNECTION Dental network, you will likely see cost
savings. The network grants you access to providers nationwide. You will
see a cost savings through negotiated, competitive discounts. To search
for providers, please visit www.umr.com or call 800.826.9781. If you ind
that your current dentist is not in the network, you will not be penalized
and will have the option to continue seeing that dentist. However, if your
dentist is in the network, you will see a cost savings due to the negotiated
discounts. If the dentist of your choice is not listed in the directory,
you may complete a form on the CONNECTION Dental website
to nominate the dentist to participate in the CONNECTION Dental
network.

when dental problems arise is important to your overall health.
Employees and their families should maintain good dental habits and
