Vision Plan Options
Vision
Vision coverage is an optional beneit for eligible full-time, exempt and
oficer employees and their dependents provided through a self-funded
plan administered by UMR. One routine eye examination per calendar
year by an eye doctor is covered at 100 percent, and eyeglass lenses and
frames or contact lenses prescribed by an eye doctor are covered at 50
percent. The maximum beneit per covered person during the calendar
year is $250 (not subject to a deductible).
7/1/15 to 6/30/16 Vision Plan
Semi-Monthly Premium
Employee only $1.85
Employee + spouse $7.78
Employee + child(ren) $6.66
Employee + family $11.85
