Your Plan’s Pharmacy Network
Your plan’s pharmacy network includes more than 64,000 independent
and chain retail pharmacies nationwide. To ind a participating pharmacy
near you, log into myuhc.com and search for a participating pharmacy, or
call Customer Service at 877.559.2955.
Mail Service Pharmacy
You may receive up to a 90-day supply of your medication delivered right
to your home. Choose from one of two options to get started with mail
service in just one easy phone call.
Option 1
Call OptumRx at the member services numbers.
Traditional PPO—866.844.4864
Consumer HSA—866.734.7670
Option 2
Talk to your doctor. Tell your doctor you want to use OptumRx for home
delivery of your preventive drugs. Be sure to ask for a new prescription
written for up to a 90-day supply with three reills to maximize your plan
beneits. You can then either complete an order form and send it, along
with your prescriptions and coinsurance, to OptumRx or ask your doctor
to call 800-791-7658 for instructions on how to fax or electronically
submit your prescription.
