How to Enroll

Qualifying Events

A qualifying event is a change in your personal life that may impact your eligibility or
dependent’s eligibility for beneits.


Examples of some qualifying events include:


„ Change of legal marital status (i.e., marriage, divorce, death of spouse)
„ Change in number of dependents (i.e., birth, adoption, death of dependent, ineligibility
due to age)

„ Change in employment or job status

It is your responsibility to contact the Employee Service Center at 866.213.8919 within 30
days of the qualifying life event. Failure to notify the Employee Service Center within 30 days
will prohibit your ability to change your beneits election(s).
