Annual Enrollment
It is time to make some important decisions about beneits for the
2016 plan year. Annual enrollment will take place from November
9 through November 22. You will not be required to make beneit
elections for 2016. However, if you choose to participate in the
FSA, you must re-elect your contribution amount. Please review
this guide to learn more about your beneits. All elections placed
during this annual enrollment period take effect January 1, 2016.

Please go to www.hcm.ininisolved.com to login to your iSolved
account.
Eligibility
Your eligible dependents include:
Legal spouse, who does not have access to his/her own
employer-sponsored health plan
Dependent children up to age 26
Legal wards; children for whom you are a legal guardian, up to
age 26
