to last year’s Annual Enrollment, if you need to change any of your current
beneits elections, you must do so on the My Four Seasons Beneits Center
website (https://fsbeneits.fourseasons.com) or call the My Four Seasons
Beneits.
If you do not complete the enrollment process, you will automatically continue
your current 2014 plan options for the 2015 plan year at the updated rates and
with plan changes.
Enrolling For Benefits
Be sure to review the Annual Enrollment Worksheet which will be sent to
your home. It includes a list of dependents we have on record as family
members eligible for coverage. It is important to correct this list to ensure your
dependents have coverage in 2015.
If you cannot access the My Four Seasons Beneits website, or if you have any
questions about enrollment or your beneits, please call the My Four Seasons
Beneits Center at 1-866-672-0435 (toll free) between 8 a.m. and 8 p.m. EST
and speak with a representative.
