Primary Care Physician (PCP)—a network doctor of your choice who
coordinates your medical care under the Medical Plan. A PCP is usually
a general practitioner, family practitioner, internist, or pediatrician. Your
PCP is responsible for referring you to specialists as needed, as well as
precertifying hospitalization.


Reasonable and Customary (R&C)—a typical fee for a service or supply
charged by comparable providers in the same geographic area. You are
responsible for payment of any charges above the R&C limit. Charges above
R&C limits do not apply toward your annual deductible or out-of-pocket
maximum.


Urgent Care—medically necessary treatment of severe symptoms which
occur unexpectedly but are not life threatening. Urgent care is appropriate
when delaying treatment could lead to serious medical problems and when
treatment is received in an urgent care center rather than in an emergency

room.
