Qualifying Life Event
A qualifying life event is a change in your personal life which may impact
your eligibility or dependent’s eligibility for beneits. Examples of some
qualifying events include the following.
X Change of legal marital status (e.g., marriage, divorce, death of spouse,
legal separation)
X Change in number of dependents (e.g., birth, adoption, death of
dependent, ineligibility due to age)
X Change in employment or job status
If you have a qualifying life event, you must contact HR within 30 days of
the event. Failure to notify HR within 30 days will prohibit your ability to
change your beneits election(s) until the next beneits enrollment period.
Benefits Eligibility
Eligible Employees
You may enroll in the beneits program if you are a regular full-time
employee who is actively working a minimum of 30 hours per week.
Eligible Dependents
As you become eligible for beneits, so do your eligible dependents.
Generally, eligible dependents include your legal or common law spouse,
domestic partner, and children up to age 26 regardless of their marital, work,
or inancial dependency status. Children may include natural, adopted, step-
children, domestic partner’s child, children obtained through court-appointed
legal guardianship, or over age 26 disabled children.
Required Documentation and Identification
All enrollees must provide marriage certiicates and documentation of
current marital status such as a joint bill or bank account for participating
spouses, and birth certiicates for all participating children. If you cover a
domestic partner or common law spouse, you will be required to complete
an afidavit to conirm their status. We ask that you provide valid Social
Security Numbers and/or tax payer ID for each enrolled dependent.
Employers are required to provide names and Social Security Numbers
(when possible) to the federal government for each individual enrolled for
medical coverage. This reporting requirement helps the federal government
verify compliance with the Health Care Reform Individual Mandate. 3
