Critical Illness

The Compass Critical Illness Insurance offered through Voya pays
a lump-sum beneit if you are diagnosed with a covered illness or
condition. The money you receive through this beneit can help you
pay for your medical expenses or any life expenses that you may have
(utility bills, mortgage, etc.). You can purchase this beneit in $5,000
increments up to $20,000. Once you purchase coverage for yourself,
you may also purchase coverage for your spouse or child(ren). The full

beneit you elected is paid out for several major illnesses or conditions
including but not limited to the following: cancer, heart attack, stroke,
major organ failure, coma, deafness, blindness, multiple sclerosis, ALS,
and Alzheimer’s disease.


Please note conditions for which you received medical treatment for
in the prior 12 months are not covered for the irst 12 months of
coverage under the policy. Following your irst 12 months of coverage,
beneits for your pre-existing condition are the same as any other
eligible condition.


The policy also includes a wellness beneit. This provides an annual beneit payment
if you complete a health screening test. You may only receive a beneit once per year,
even if you complete multiple health screening tests. Examples of a health screening
test include but are not limited to: Pap test, serum cholesterol test for HDL and LDL
levels, mammogram, colonoscopy, and stress test on bicycle or treadmill. The annual

beneit is $50 for completing a health screening test. If your spouse and/or children
are covered, they are also eligible for the wellness beneit. Your spouse’s beneit
amount is also $50. The beneit for child coverage is $25 with an annual maximum
of $100 for children’s beneits.
