Accident
The Compass Accident Insurance offered through Voya pays you beneits for
speciic injuries and events resulting from a covered accident. Examples include,
but are not limited to: hospital care and emergency care for fractures, dislocations,
burns, lacerations, surgeries, therapy, etc. Under this policy, you do not elect a certain
coverage amount, but rather you elect to be covered and the beneit amount depends
on the type of injury and care received. You have the option to enroll you, your
spouse, and/or your child(ren), similar to the medical, dental, and vision plan.
Just like the critical illness plan, the money you receive through this beneit can help
you pay for your medical expenses or any life expenses that you may have (utility
bills, mortgage, etc.). Coverage is guarantee issue—no questions asked. There are no
pre-existing condition limitations. This policy also includes a wellness beneit, just
like the critical illness policy.

Employee (Ee) only $14.01
Ee + spouse $24.49
Ee + child(ren) $29.32
Family $39.80
