As we said at the front of this Guide, your health and well-being matters. Well-being is a positive outcome
meaningful for all of us because the pursuit of well-being means our lives are going well.

Well-being Program Activities and CommScope Rewards



We know that well-being is more than physical, that’s why we’ve designed a program that includes physical,
emotional, and inancial well-being resources. All CommScope employees are invited and encouraged to
participate in our well-being program and all CommScope employees can receive rewards in different ways.
When you complete various activities, you receive “Rally Coins,” which is digital currency you can use to purchase
discounts and tangible rewards on our wellbeing platform.

Active employees enrolled in our Health & Investment Plans A or B with Health Savings Account (HSA) through
Bank of America can receive up to $500 CommScope paid dollars into their HSA in 2019 with employee only
coverage and up to $1,000 CommScope paid dollars in 2019 with employee + dependent coverage, when they
complete speciic well-being activities by October 31, 2018.

Up to $500 will be contributed to active employee participant HSAs in January 2019, with the remaining earned
funds contributed for active employee HSA participants in July 2019 if the active employee is then eligible to
contribute to an HSA.
