Short Term Disability (STD) Coverage

To help protect your financial stability in the event you became ill
or disabled, Advia CU provides a salary continuation plan through
a short-term disability program.

Short Term Disability (STD) provides income protection in the
event you become temporarily disabled due to a non-work related
illness or injury. (If you remain disabled when your STD benefits
end, you may be eligible for LTD benefits which are outline on the
following page.) Both of these benefits are provided through Sun
Life.

Your STD benefits are outlines as follows:

STD Benefit

Weekly Benefit 66 2/3 % of Pre-Disability Earnings

Minimum/Maximum Weekly Benefit $20.00 / $1,200 per week
Elimination Period
Benefit Duration 8 days for accident
8 days for sickness

12 weeks Duration

Pre-Existing Condition Limitation None
Definition of Disability
Due to sickness, or as the result of an accidental injury:
- is receiving Appropriate Care & Treatment and
- complying with the requirements of such treatment, and
- is unable to earn more than 80% of their pre-disability
earnings at their Own Occupation for any employer.

Long Term Disability (LTD) Coverage

Long Term Disability (LTD) provides income to employees who are
disabled for an extended period of time. The LTD plan is designed
to help protect your income in the event that you are unable to work
due to a covered disability.

Your coverage effective date or any increase in coverage may be
delayed if you are disabled on the date coverage is scheduled to
take effect. Review the Summary Plan Description for details.
