LTD Benefits

Monthly Benefit 60% of Pre-Disability earnings

Elimination Period 90 Days or the end of the STD Maximum Benefit Period

Minimum/Maximum $100 / $10,000 per month
Monthly Benefit The later of Your Normal Retirement Age as defined by Social Security or the

period shown below:

Due to a sickness, or as a direct result of accidental injury:

Definition of Disability - The employee is receiving Appropriate Care and Treatment and
Pre-existing Conditions complying with the requirements of such treatment, AND

- During the elimination period and the next 24 months is unable to earn
more than 80% of their pre-disability earnings at their Own Occupation for
any employer in their Local economy, AND

- After such period, is unable to earn more than 60% of their pre-disability
earnings from any employer in their Local economy at any gainful
occupation for which they are reasonably qualified taking into account
their training, prior education and experience.

3 / 12 Rule – Benefits are not payable for any disability that is caused by, or
contributed to by a pre-existing condition during your first twelve (12) months of
coverage. A disease or injury is pre-existing if, during the three months before
your coverage effective date, (1) it was diagnosed or treated, or (2) services were
received for the diagnosis or treatment, or (3) you took drugs or medicines
prescribed by a physician for the condition.
