How to enroll
Making your decisions, adding it all up, and moving forward
1. Do I enroll, and who do I cover? | Review this benefits guide: enrollment is simple!
IMPORTANT: Any employee waiving 2017 medical coverage must log in and select waive and enter the reason for
waiving medical coverage due to the Affordable Care Act (ACA) reporting requirements.
2. How do I enroll? | https://home.eease.com/login
Step 1: Go to https://home.eease.com/login
– As you enter the system, the first screen you will see will be the Data Update Wizard.
– Review your demographic information and family member information.
• PLEASE MAKE SURE ALL OF YOUR DEPENDENTS’ SOCIAL SECURITY NUMBERS ARE IN THE SYSTEM.
• Verify that your dependent(s) meet the eligibility requirements.
Step 2: The process is simple—especially if you choose “Walk me through this process.”
If you have any questions about Open Enrollment, how to use our online benefits enrollment system or any other aspect
of our benefit plans, if you need your password reset, please e-mail mybenefits@epicor.com. Mayra Palascio assists employees
with a last name beginning A - K. Lilia Garza assists employees with a last name beginning L - Z.
3. Do I set aside any additional dollars in an FSA? | REMINDER! You must re-enroll every year if you want to continue
participating. If you enroll in the HDHP HSA medical plan, you may enroll in a Limited Purpose Flexible Spending
Account (LPFSA).
4. Do I set aside any additional dollars in the 401(k)? | Refer to page 14 for information on this important decision. You can
enroll in your 401(k) by going to netbenefits.com. If you have any additional questions, please contact the Benefits Department at
mybenefits@epicor.com. There is no time limit to enroll.
5. What’s next? | First, sit back and congratulate yourself on making good decisions for you and your dependents! Next, focus your
